Blue Cross of Jamaica Limited, in partnership with Jamaica Money Market Brokers, now offers three attractive health insurance options to JMMB clients covering medical, surgical, drugs, dental and optical health care services.

Service Benefits -

• Claims processed within 15 working days of reaching Blue Cross®
• Access to Blue Cross® Wellness Services and the Mobile Unit. *Ask about our Wellness Discount programmes with health care facilities such as spas and gyms.
• Once you become a Blue Cross® member before the age of 65 years, you remain a member thereafter providing you pay your monthly premiums

Eligibility:

• All JMMB clients are eligible.
• You may apply for individual coverage if you are between 18 and 65 years of age.
• You may also opt for family coverage for your spouse (married or common-law) and/or dependent children (legal or adopted) up to 18 years of age. Children ages 19 to 22 may be covered provided they are enrolled at a tertiary institution as a full-time student.

Enrolment:

All enrollment transactions are channeled through JMMB Branch offices. After selecting your BlueCare Health Plan Option:
1. Complete and sign a Blue Cross® Application Enrolment Card along with authorization to deduct your premium at any JMMB office.
2. On receipt of your application and premium, Blue Cross® will prepare your health card which you will collect from your JMMB branch.
3. The card will be valid until the end of the policy year and may be renewed annually.

Once you’ve received your health card, you are eligible for immediate benefits, except where waiting periods apply –

• Pregnancy – any related condition / maternity benefits – 9 months
• Pre-existing Medical and Surgical Conditions - known or unknown – 6 months
• Hospitalization and Services - paid for major medical - 6 months

Adjustments to enrolment status:

• If you are on an Individual plan, you may transfer to Family membership at anytime during the year. Any stipulated waiting periods will commence form the effective date of the transfer of dependents.
• Newborns will be registered with the plan 14 days after birth. If the newborn is hospitalized on the eligibility date, the effective date will be that of the date following discharge from the hospital. If the employee is enrolled on a family plan, the newborn will be added from the date of birth.
  
  • In the case of adoption, adoption papers are required or proof of legal Guardianship (letter signed and stamped by Justice of the Peace confirming care of child and processing of legal guardianship).
  • Step Children (children of spouse covered by the plan) will be covered up to age 19.
• Changing your spouse on your plan can be done only on the anniversary date of the contract. If marriage occurs, the change of spouse can be effected during the policy year.

Access to Benefits:

Blue Cross of Jamaica Limited has the widest network of providers islandwide i.e. doctors, dentists, opticians, pharmacies, diagnostic services, hospitals.

When you visit a Blue Cross® participating provider, present your health card and claims filed on your behalf will be settled by Blue Cross® in favour of the provider, in keeping with your benefits.

If you don’t have your Blue Cross® card, the provider will charge you the normal fee. Complete and sign your Blue Cross® claim form and submit along with original receipts to Blue cross of Jamaica Limited within 90 days of service. You will be reimbursed in accordance with your schedule of benefits.

Premiums:

Your premiums are due on the stated date as per your contract and must be paid by the date specified by JMMB to avoid the cancellation of your policy. Premiums are stated according to the selected option and Age band.
