JN Blue is designed to provide a broad range of health benefits to all members of JNBS, their spouses (married or unmarried) and children who may not be eligible for coverage under an employee group plan, or for those persons who wish to supplement their existing coverage.

JN Blue offers coverage for hospital surgical and other medical expenses for the insured and their dependents. Benefits include:

• medical,
• dental and optical visits,
• major medical,
• hospital services,
• surgery,
• prescription drugs,
• maternity coverage,
• physiotherapy, radiotherapy and
• overseas emergency coverage

ELIGIBILITY

JN members between the ages of 18-69 years and their

• Spouse (married or unmarried)
• Dependent children up to the age of 19 (If your dependent child is a full time student at a tertiary institution, coverage continues to age 23.)

OPTIONS

* JN Blue Basic - An affordable full benefit package
* JN Blue Premium - Full coverage, plus overseas benefits
* JN Blue Advance - Coverage in the prime years of your life (age 55-69)

ENROLMENT

1. Visit any JNBS Branch. Choose the option that's right for you, and then complete and sign a Sagicor Application/enrolment form along with a standing order deduction form. Premiums will be deducted from your JN Account. (If you are not already a JNBS member, you need to open an account.)

2. Jamaica National will submit your application form to Sagicor Life Jamaica Limited for processing

3. Sagicor will review your application, and then prepare and submit your JN Blue membership card to your Jamaica National branch.

Your JN Blue membership is valid for one year as long as your monthly premiums are paid. Your card is renewed annually.

Note: Newborn babies and legally adopted children should be added to your plan within 30 days of birth and adoption.

WAITING PERIODS

• Pregnancy Nine (9) months for maternity benefits and benefits for any condition related to pregnancy.
• Surgery Six (6) months whether the condition is known or unknown.
• Pre-existing medical conditions Six (6) months for such conditions, whether known or unknown.
• Hospitalization and services which go into Major Medical Six (6) months.
• Change of spouse This is done only on the anniversary date of the contract or where marriage has taken place.

HOW TO ACCESS BENEFITS

Your benefits commence from the effective date of your health policy. For services, you are required to present your membership card to the provider. If the provider is a Sagicor participating provider, you will only need to pay the required co-payment amount and sign the completed provider form.

Your JN Blue plan covers 80% of the cost of your prescription drugs, dental and optical expenses. You pay the balance until you exhaust the credit limit under the JN Blue plan that you choose. For prescription drugs, after the deductible is satisfied, the benefit goes into major medical on a reimbursable basis.

Your Jamaica National branch will provide you with a full schedule of the benefits of your JN Blue plan.
