If you feel the need for additional health coverage for you and your family, then the Exclusive Blue Supreme plan is a perfect supplement to your existing health insurance plan.
As a subscriber to this plan, you are entitled to a comprehensive array of benefits and services not afforded to those enlisted in other plans we offer.
Additionally, Exclusive Blue Supreme also carries an overseas emegency benefit for medical care.
COVERAGE
If you are without health insurance or wish to add to your existing health care benefits, you need health coverage at a price that you can afford. Blue Cross of Jamaica Limited's line of Independent Health Coverage Plans offers you a range of benefit options to satisfy your personal and family health needs and budget.
When you select the Family Coverage, you do not have to worry about the health care needs of your spouse, whether married or un-married, and your children. Under an individual health plan, the health care benefits apply only to you.
BENEFITS
Choose from two plan options, which cover a range of medical services:
If you are on a Blue Cross® Group Plan and terminate your employment, you can continue your coverage, by transferring to one of these plans within thirty (30) days of the termination of your employment. You would not be required to pass a medical and there is no waiting period for pre-existing conditions.
You may increase your coverage at any time. However, you may need to provide new evidence of insurability.
The Exclusive Blue Supreme, Executive Blue and Executive Blue Plus Health Plans allow for the coordination of benefits where you, or your dependents are already covered under more than one health plan.
ENROLMENT
PREMIUMS
Your premium should be paid on the due date, or within thirty (30) days of that date. Avoid policy cancellations by ensuring that your premiums are paid on time each due date.
You have two (2) premium payment options:Monthly premiums are allowed only if the premium is being paid through your company via salary deduction under the Individual Health Coverage Plans. If you opt to pay monthly premiums yourself you will be required to pay for your medical services and submit claims to and for reimbursement by Blue Cross of Jamaica Limited.
Premiums paid on an annual basis will receive a discount.

HOW TO ACCESS BENEFITS
To access benefits, you must show your Blue Cross® Membership Card to the provider - doctor, pharmacy, laboratory and diagnostic service provider.
CLAIMS
You must submit claims within 90 days or service. Blue Cross® reserves the right to deny payment of claims submitted after 90 days.
You must affix your membership number, and correct name and address as failure to do this will result in your claim not being processed.
You should also state your Blue Cross® number on all correspondence you send to Blue Cross®, or if you call the office with a query.
